Q. R. Adequate expansion of the osseous crest allows placement of osseointegrated implants in a more coronal position than would be normally possible. The objective is to place the fixture head so that the anticipated gingival attachment to the implant will be at a similar level as the periodontal attachment to the adjacent teeth. The distraction objective is to expand the alveolar housing to provide a site for implants or to create additional tissue for a second stage reconstructive procedure. PRINCIPLE: An osseointegrated fixture adjacent a tooth functions best when their respective gingival attachment mechanisms are at the same vertical height.
S. The osseointegrated fixtures are positioned relatively coronal in the alveolus. To remain stable, the anticipated gingival attachment on the implant fixture should minimally impact the periodontal attachment on the adjacent teeth. Proper positioning of the implant is important in achieving this goal. Adequate site reconstruction is often a prerequisite to satisfactory implant positioning. The biologic width on the implant will probably be similar to that of the tooth.
